An unannounced, on-site monitoring inspection was initiated and completed on 10/28/2024. The on-site inspection began at noon and ended at 1:30.The inspector reviewed compliance in the areas listed above. There were 27 children present and 5 staff.
The inspector reviewed 5 children?s records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Centre are required to have a flashlight and a working battery operated radio.

The center did not have a flashlight or a working battery operated radio.
Plan of Correction: The supplies will be ordered and placed in the classroom.
Standard 8VAC20-780-60-A
Child records are required to have two emergency contacts with addresses and a proof of ID within 7 days of attendance.

Child #2 and #5 did not have two addresses for their emergency contacts. Child #5 did not have proof of ID.
Plan of Correction: Center staff will get the information from the parents.
