Inspector Notes
An unannounced renewal inspection was conducted on 3/8/2023 from 11:45am to 1:20pm. There were 68 (5 months to 5yrs) directly supervised by 8 staff. The physical plant, 2 staff records, 5 children?s records, 0 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed toddlers to pre-k students taking naps, and the infants were either napping or interacting with staff. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. 2Evidence: observed that the diaper changing surface had several small rips and tears exposing the absorbent surface beneath.
Evidence: observed diaper ointment in a small cubby above the changing table that was not labeled with a name.
