Inspector Notes
An unannounced complaint inspection was conducted on 9/13/2023 to investigate an allegation received by the department related to supervision of children. Based on evidence gathered through interviews, photos and documentation review, the complaint was determined to be valid. A violation notice was issued to the facility. Based on interviews, photos and documentation review it was determined that the facility failed to ensure the children's care, protection, and guidance. Evidence: 1) in July 2023, there were approximately 22 documented accidents/incidents in the two's classroom including four bites, four scratches, and six bumps/hits. 2) in August 2023 there were approximately 20 documented accidents/incidents in the two's classroom including seven bites and eight scratches/hits. 3) interviews revealed that there were staffing issues and the unusually high number of incidents have been rectified with new/consistent staffing.
