Inspector Notes

An unannounced monitoring inspection was conducted with the local department of social services and law enforcement on 6/3/2024 to investigate a self report related to Staffing and Supervision, Programs, and Special Care Provisions and Emergencies. Additional interviews were conducted on: 5/30/24 and 6/12/24. Based on evidence obtained through interviews, observations, digital documentation and record review, the self reported incident could not be substantiated. There were no violations issued as a result of this inspection. An exit interview was conducted on 6/27/2024.
