Inspector Notes
An unannounced inspection was initiated on 03/03/26 and completed on 03/04/26 in response to a complaint received by the licensing office on 01/05/26 relating to allegations of administration, staff qualifications and training, programs, special care provisions and emergencies, and special services. The inspector reviewed three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
