Inspector Notes

An unannounced monitoring inspection was conducted on 03/07/2025, as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. The inspector reviewed 5 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature and shall be intended to redirect children to appropriate behavior and resolve conflicts. In the 4 year-old room Staff A used directives and threats towards the children for not laying on their cots during nap time. Staff A made the following statements to children in her care:
"Get your butt on your cot."
"Don't stand on your cot."
"Do I need to take your stuffy away?"
"You don't get your toys today."
Plan of Correction: The director intervened and corrected the staff. The director will retrain staff on using positive redirection.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Infant staff used her hand to open the lid of the foot operated step can to dispose of a soiled diaper.
Plan of Correction: Diapering and handwashing will be reviewed with all staff.
