Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. An electrical outlet in the two year-old room did not have a cover.
Plan of Correction: This was an oversight. The electrical outlet was covered during the inspection.

The center is required to maintain all required documentation for each staff person.
1. Staff #2 did not have documentation of her qualifications required by her job title as a program leader.
2. Staff #3 had 1 reference checked prior to employment, when 2 are required.
