Each building that houses a child day program that is licensed, that serves preschool-age children, shall be equipped with at least one carbon monoxide detector.

There was no carbon monoxide detector in the building.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

The list of allergies, sensitivities, and dietary restrictions in each room were not dated.
Plan of Correction: Allergy lists without date/out of date have been removed from the entire center and replaced with alerts properly labeled with the last update 8/11/25.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child A, enrolled for two years, was missing documentation that the parental written agreements were reviewed. The record for Child A was also missing documentation of viewing proof of the child's identity and age.
Plan of Correction: Parent of child missing parental agreement and documentation of identity and age has been contacted and agreed to meet with director Thursday 2-5-26 to remedy.

REPEAT VIOLATION
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

In classroom three there was one child present, but four children were marked present on the attendance record.
Plan of Correction: Director has retrained staff on attendance tracking and will continue to monitor that priority be given to paper attendance before digital attendance. We are developing better habits.
