Inspector Notes
An unannounced monitoring inspection was conducted today from 9:08am to 10:20am. There were 42 (Infants to 5yrs) directly supervised by 10 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, and participating in circle time. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. 2Evidence: Reviewed 4 staff records and found that 2 (Staff A and B) did not have documentation of a sworn statement or affirmation.
Evidence:
1) Observed in the Toddler Room a mirror along the wall that was loose, causing a pinching hazard.
2) Observed on the Toddler Playground, playground anchors that were exposed and not covered.
3) Observed in the Pre-K classroom an unlocked lower cabinet with dish soap accessible to children.
4) School-Age classroom utility closet was unlocked with paint, ice melt and cleaning agents.
5) Crack on the playground bench causing a pinching hazard.
