Based on review, the facility did not ensure that ensure that they obtained documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Reviewed 2 children's record and found that Child 2 did not have documentation of an immunization record. The child has been in care for over 12 months.

Based on review, the facility did not ensure that each child has documentation of a physical examination prior to the first day of care, or within 30 days after the first day of attendance.
Evidence: Reviewed 2 children's records and found that Child 2 did not have documentation of a physical examination. Child 2 has been in care for over 12 months.

Based on staff interviews and review of children's records, the facility was not ensuring the care, protection and guidance of children in care.
Evidence: Reviewed 2 children's records and classroom bite logs and found that Child 1 had bitten 14 children, was bitten 10 times. During the month of May they had 21 attempted bites and 3 contact bites. The facility has policies in place to disenroll children that bite excessively with no improvement, but they did not follow their own policy, and allowed the child to remain in the Toddler classroom, before moving the child to the Twos classroom where they continued to bite other chidlren. Documentation of the child's biting habits started in June of 2021, the month after the child was enrolled into the facility.

Based on review, the facility did not ensure that there was a program lead qualified staff member assigned to the Toddler Room.
Evidence: Reviewed 2 staff records and found that neither staff member was program lead qualified, and that the facility recognizes both staff members as assistants. Neither of the two staff members assigned to the Toddler classroom were program lead qualified.

Based on review, the facility did not ensure that documentation of the parent's written authorization to move a child to a different age group, was maintained in the record.
Evidence: Reviewed 2 children's record and found that Child 1 did not have documentation of written parental permission to move the child to a different age group.

Based on observation, the facility did not ensure that there shall be at least 12 inches of space between occupied cots, beds and rest mats.
Evidence: observed in Twos classroom, four occupied cots that were less than 12 inches apart.

Based on observation, the facility did not ensure that over-the-counter skin products were labeled with the child's name.
Evidence: observed in the Two's classroom a diaper ointment bottle that was not labeled with a child's name. Interviewed staff and found that it belonged to a specific child in care, and had not been labeled.
