Inspector Notes
An unannounced complaint inspection was conducted on 5/20/22 in response to a complaint that was received by the Virginia Department of Education, Office of Child Care Health and Safety on 4/10/2022. Interviews were conducted with staff relating to allegations supervision, care and protection, programing, policies. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of supervision and programming, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Violations
7Evidence: Reviewed 2 children's record and found that Child 2 did not have documentation of an immunization record. The child has been in care for over 12 months.
Evidence: Reviewed 2 children's records and found that Child 2 did not have documentation of a physical examination. Child 2 has been in care for over 12 months.
Evidence: Reviewed 2 children's records and classroom bite logs and found that Child 1 had bitten 14 children, was bitten 10 times. During the month of May they had 21 attempted bites and 3 contact bites. The facility has policies in place to disenroll children that bite excessively with no improvement, but they did not follow their own policy, and allowed the child to remain in the Toddler classroom, before moving the child to the Twos classroom where they continued to bite other chidlren. Documentation of the child's biting habits started in June of 2021, the month after the child was enrolled into the facility.
Evidence: Reviewed 2 staff records and found that neither staff member was program lead qualified, and that the facility recognizes both staff members as assistants. Neither of the two staff members assigned to the Toddler classroom were program lead qualified.
Evidence: Reviewed 2 children's record and found that Child 1 did not have documentation of written parental permission to move the child to a different age group.
Evidence: observed in Twos classroom, four occupied cots that were less than 12 inches apart.
Evidence: observed in the Two's classroom a diaper ointment bottle that was not labeled with a child's name. Interviewed staff and found that it belonged to a specific child in care, and had not been labeled.
