During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6-16-25, as a part of the provisional licensure period. The on-site inspection began at 11:30am and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 10 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/23/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
(Repeat Violation) The Center is required to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

On the large playground, there are 5 large roots protruding from the area on the back of the playground by the fence which could pose a tripping hazard.
Plan of Correction: All identified roots have been removed from the playground area.
Standard 8VAC20-780-340-A
The Center is required to ensure the care, protection and guidance of all children in care.

During inspection in the Twos classroom, 2 children were standing on top of shelves looking out of the window. Staff did not provide any guidance to the children in refraining from standing on top of the shelves until the inspector asked staff to intervene. There was a third child in the classroom who was climbing on top of a shelf and jumping off, and there was a fourth child in the classroom who was climbing on top of a shelf and sitting inside of a basket on the top of the shelf.
Plan of Correction: Management has addressed the incident with the Twos classroom staff and reviewed supervision expectations. Staff have been retrained on active supervision and immediate redirection of unsafe behaviors. Management will monitor classrooms daily to ensure staff are consistently guiding and protecting children.

(Repeat Violation) The Center is required to maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that is accessible to all staff in each group or area where children are present.

During inspection the list of allergies, sensitivities and dietary restrictions did not contain one of Child #1?s allergies, sensitivities or dietary restrictions.
Plan of Correction: Management will ensure that a current and dated allergy list is maintained and accessible in all classrooms and food preparation areas. All children's allergies, sensitivities, and dietary restrictions will be reviewed and updated weekly by management. A copy will be posted in each classroom and kitchen area.
Going forward, any updates provided by families will be added prior to the child's next date of attendance and initialed by management. Compliance will be verified weekly.
Standard 8VAC20-780-80-A
The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

1)During inspection in the Infant classroom, there were 7 children present, but the written record reflected there were 6 children present.
2)During inspection in the PreK classroom, there were 15 children present, but the written record reflected there were 14 children present.
