Inspector Notes

An unannounced renewal inspection was conducted on 04/25/23 from 11:15 a.m. ? 3:16 p.m. with the Director and Assistant Director. There were 72 children in care, ranging in age from 3-months to five-years-old, supervised by twelve staff. The children were observed playing outside, preparing for lunch and napping. For lunch the children had spaghetti with meat sauce, peaches, broccoli and milk. Nine child records and five staff records were reviewed. Two medications and their authorization forms were reviewed. The attendance, menu and emergency drill log were reviewed. The most recent evacuation drill was dated 3/7/23. The most recent Fire Inspection on file was dated 11/4/22 and the most recent Health Inspection on file was dated 1/9/23. Based on review of five staff records, the center did not obtain documentation of the results of a national fingerprint-based background check prior to employment for one staff.

Evidence: The record for Staff B, with a employment date of 10/17/22, contained documentation of anational fingerprint-based background check dated 10/25/22.

Based on review of five staff records, the center did not obtain documentation of a completed sworn disclosure statement for one staff prior to employment.

Based on review of five staff records, the center did not obtain documentation that one staff had completed the Virginia Department of Education ? sponsored orientation (Preservice Training) within 90 calendar days of employment.

Evidence: The record for Staff E, with an employment date of 01/10/23, did not contain documentation of Preservice Training.

Based on observation, the center did not ensure that hazardous substances such as cleaning materials, were kept in a locked place.

Evidence: On 04/25/23, in the toddler classroom, the Licensing Inspector?s observed a spray bottle bottle labeled bleach and water spray in an unlocked cabinet located just below the diaper change station.

Based on observation and interview, the center did not ensure that infants were
placed on their back in a crib.

Evidence: On 04/25/23, the Licensing Inspector?s observed Child D, eight months old, sleeping on their stomach. A staff member revealed that they had placed Child D on their stomach when they placed the child in their crib.

Based on review of nine child records, the center did not obtain all of the required documentation for each child.

Evidence: 1.The record for Child B, with a start date of 02/26/18, and Child C, with a start date of 5/31/22, did not contain documentation of viewing proof of the child?s identity and age.
