Inspector Notes
An unannounced renewal inspection was conducted on 04/25/23 from 11:15 a.m. ? 3:16 p.m. with the Director and Assistant Director. There were 72 children in care, ranging in age from 3-months to five-years-old, supervised by twelve staff. The children were observed playing outside, preparing for lunch and napping. For lunch the children had spaghetti with meat sauce, peaches, broccoli and milk. Nine child records and five staff records were reviewed. Two medications and their authorization forms were reviewed. The attendance, menu and emergency drill log were reviewed. The most recent evacuation drill was dated 3/7/23. The most recent Fire Inspection on file was dated 11/4/22 and the most recent Health Inspection on file was dated 1/9/23. Evidence: On 04/25/23, in the toddler classroom, the Licensing Inspector?s observed a spray bottle bottle labeled bleach and water spray in an unlocked cabinet located just below the diaper change station.

Evidence: On 04/25/23, the Licensing Inspector?s observed Child D, eight months old, sleeping on their stomach. A staff member revealed that they had placed Child D on their stomach when they placed the child in their crib.
Evidence: 1.The record for Child B, with a start date of 02/26/18, and Child C, with a start date of 5/31/22, did not contain documentation of viewing proof of the child?s identity and age.
