Inspector Notes

A complaint investigation was completed with the center on 1/18/2024. The complaint consisted of allegations received by the Northern Licensing Office regarding supervision, inside temperature, and behavioral guidance. Interviews were conducted, documentation was reviewed and observations were made in each classroom. The allegations regarding supervision and inside temperature are determined to be valid. The allegation regarding behavioral guidance was unable to be substantiated due to insufficient corroborating evidence. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the center did not ensure that the temperature in areas occupied by children is maintained no lower than 68?F.

Evidence:
1. In the Preschool area, the wall-mounted thermostat had a reading of 65?F.
2. In the Toddler room, thermometer reading showed a temperature of 65?F.

Based on observation, interview, and record review, the center did not ensure that the required ratio of one staff to four children was followed for children ages birth to 16 months.

Evidence: On 1/18/2024, at approximately 10:20am, there were six children in the Toddler classroom with one staff. The youngest child in the classroom was 13-months-old.
