During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on 10/28/2024 in response to a complaint received by the licensing office on 10/21/2024 relating to allegations of staffing and supervision, special services, programs, physical plant, and administration. The inspector reviewed 3 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegations regarding programs and staffing and supervision; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/14/2025. A POC submitted after this date will not appear on the public website.
Violations
4The required ratio for children from birth up to age 16 months is one staff per four children.
On 10/16/24 the Infant room was out of ratio for a period of 8 minutes in one instance (five children with one staff) and 18 minutes in another instance (nine children with two staff) between 7:00am and 8:05am. The youngest child in the room was six months old.
On 10/21/24 the Infant room was out of ratio for approximately 30 minutes with six children present with one staff and the youngest child in the room was 15 months old.
On 10/21/2024 the Toddler room was out of ratio for approximately five minutes with six children to one staff with the youngest child being 16 months old.
On 10/17/2024 the Toddler room was out of ratio for approximately 35 minutes with nine children and one staff with the youngest child being 19 months old.
On 10/22/2024, the Fours classroom was out of ratio for approximately three and a half hours. There was one staff with 11 children and the children were four years old.
On October 17, 2024, Staff #1 yanked Child #3 (Age 3) off the ground by one arm roughly and asked "What are you doing?" when called to the classroom to assist with the child's behavior.
