Inspector Notes
An unannounced monitoring inspection was initiated and completed on 1/16/2025 in response to a self-report received by the licensing office on 1/8/2025 relating to supervision. The inspector reviewed 1 child?s record on 1/16/2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/23/2025. A POC submitted after this date will not appear on the public website.
Violations
1Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 1/3/2025, Child #1 (Age 2) was out of sight and sound supervision of staff for approximately 20 seconds when Staff #1 turned their back to the classroom to prepare for naptime. Child #1 left the Preschool room and walked to the other side of the building to the Twos room.
? Always maintaining appropriate child-to-staff ratio to ensure adequate supervision of the children based on their age and development needs.
? Staff should position themselves in clear visibility and audibility of all the children in their care and the classroom to avoid any blind spots.
? Minimize distraction by avoiding activities that may distract them from supervising children.
? Constant monitoring of the environment to ensure all children are safe and accounted for.
? Enforce company supervision policy especially during high-risk periods such as naptime, transitions, mealtime, and arrival/departure time.
Staff #1: Staff was coached and retrained one-on-one on proper sight and sound supervision on 1/08/2025.
Child #1: Child?s parent was notified by phone call and an incident report was provided and signed by the parent.
