During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 4/3/2025 in response to a complaint received by the licensing office on 3/31/2025 relating to allegations of supervision, ratios, staff qualifications, injury reports, and care, protection, and guidance. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to injury documentation and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/26/2025. A POC submitted after this date will not appear on the public website.

The center is required to maintain a written record of children's serious and minor injuries that includes date and time when parents are notified and contains staff and parent signatures or two staff signatures.

The injury report dated 2/25/2025 for Child #6 did not include the date and time the parent was notified of the injury and only contained one staff signature and no parent signature.
Plan of Correction: -All staff members were re-trained on incident reporting protocol on April 28, 2025. Incident reports will be reviewed daily for accuracy and completion by a member of management. An incident report binder has been created to ensure compliance.
