The center shall obtain for each staff a Sworn Disclosure Statement disclosing whether they have ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether they have been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

Plan of Correction: This will be implemented and required as part of a new hire onboarding required documents. I will see to it that it gets signed and uploaded into the staff document portal on our company system and filed into their staff record on the first day of employment.

Each staff member shall submit documentation of a negative tuberculosis (TB) screening prior to employment.

-In compliance with licensing regulations, a new processing system by requiring all new hires to submit a central registry search request as part of the new hire process, along with fingerprint, TB requests etc.
Moving forward, no staff member will begin their first day of employment without submitting all
employment documentation and completing any necessary state training requirements. I will ensure this
by meeting with all new hires prior to their first day to verify all required documentation is completed and
current.

The center shall ensure staff records contain all required information, including documentation that the individual possesses the education, experience, and orientation as required by the job position as well as documentation that two references were checked prior to hire.
Staff #1, employed for three months, did not have documentation of qualifications for the position, orientation training, and the references checks on file were obtained one week after the day of hire. Staff #3, employed for three months, had reference checks on file that were obtained one day after hire.
