During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 1/31/25 in response to complaints received by the licensing office on 12/19/24 and 12/20/25 relating to allegations of program activities. The inspector reviewed five children?s records and two staff records on-site and digitally. This inspection included document review, a tour of the facility, observations, and interviews. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Though not a part of the original complaint, a violation that was not reported were found related to behavioral guidance and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/13/25. A POC submitted after this date will not appear on the public website.

A daily schedule shall be posted and allow for flexibility as children?s needs require.
There was no schedule posted in a two-year-old classroom. Following lunchtime, children were crying to lay down for nap and falling asleep on the floor while Staff #1 cleaned and told children they would need to wait to nap until all diapers were changed.
Plan of Correction: Daily schedules have been posted in all classrooms (for the parents and visitors to see) showing the order of activities that happen in the classroom throughout the day.
Standard 8VAC20-780-400-A
To promote a child?s emotional well-being, staff shall interact with the child to provide needed comfort and support.
In the two-year-old classroom, a child cried and sobbed off and on for a period of approximately 30 minutes before Staff #1 properly addressed their distress.
Plan of Correction: On 1/31/25 management reviewed with two classroom staff VDOE and LCG's expectations regarding how to handle children's emotional well-being as well as promptly addressing a child's needs through verbal or physical attention.
