During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 6/20/25. The on-site inspection began at 12:25pm and ended at 3:05pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and eight staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/1/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-260-B
Annual approval from the health department shall be provided.
There was no documentation at the center to show that a health inspection was completed in 2024, which is required annually.
Plan of Correction: Center management requested their 2024-2025 annual health inspection from the City of Virginia Beach, however the health inspector has yet to complete the annual inspection. A second request was submitted to the City of Virginia Beach for the annual health inspection on 6/27. Management will follow up weekly if no response received.

Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was air freshener located on top of a toilet in two restrooms that children use. The air fresheners were not locked and the labels stated "keep out of reach of children."
Plan of Correction: Air freshener items were immediately removed from the bathrooms during the inspection. Center Management completing a retraining with all staff regarding VDOE standard and LCG policy on hazardous substances and the expectation to keep locked away at all times. Management will monitor daily during classroom checks.

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided: the center has obtained written authorization from a parent or guardian.
The written authorization for medication for Child #1 was not signed by a parent or guardian, which is required. Medication was administered on two days in the month of June.
Plan of Correction: Management received updated medication authorization form signed by parent on day of inspection. Moving forward, Management will be on alert when reviewing medication authorizations to ensure it includes parent?s signature along with physician?s signatures.
