Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 04/01/21 and concluded on 04/05/21. The director was contacted by telephone to initiate the inspection. There were 24 children present and three staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy
regulations, and violations were documented on the violation notice issued to the facility.
Violations
2Evidence: 1) The record for Staff #1, whose hire date was 07/15/2019, does not have a completed Virgina Preservice Training for Child Care Staff on file.
2) During interview, a member of management confirmed Staff #1 has not completed the training to date.
Evidence: 1) The most recent Annual Health and Safety Update Training in the record for Staff #2, hired on 01/28/88, was completed on 10/31/19.
2) The most recent Annual Health and Safety Update Training in the record for Staff #3, hired on 09/11/06, was completed on 06/26/19.
3) During interview, a member of management acknowledged Staff #2 and Staff #3 have not completed the required annual training to date. Completion of the Annual Virginia Health and Safety Update training for child care staff is required annually of subsidy vendors and their staff.
