Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 11/04/2021 and concluded on 11/05/2021. The director was contacted by telephone and a virtual inspection was conducted. There were no children present at the time of the inspection. The inspector reviewed compliance in the areas of administration, physical plant, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
1Evidence: 1) The most recent tuberculosis (TB) screening results in the record for Staff #1, hired on 10/11/2017, were dated 12/19/2018. The TB expired on 12/19/2020. 2) During interview, a member of management confirmed this is the most recent TB screening on file for Staff #1. Staff should resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
