Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was conducted in response to a self-report made by the center in the area of programs. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and record review the center did not ensure staff did not use physical punishment.
Evidence:
On 12/07/2021, After one year-old Child 1 pushed a chair, Staff A used her fingers to tap Child 1 on the forehead, causing the child to fall to his bottom. The child's parent witnessed the situation watching through the classroom window. Staff A confirmed to the director that she had used her fingers to tap the child's forehead because he pushed a chair.
Plan of Correction: The following correction action was provided by the director during the exit interview:
The staff was terminated immediately, as the center has a "no tolerance policy." In addition, the director held meetings with staff about behavioral guidance policies.
