Inspector Notes
A monitoring inspection was conducted on 05/10/2024 in addition to an investigation in response to a self-reported incident regarding allegations in the area of behavioral guidance. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence:
1. It was reported by multiple staff that they have heard Staff #2 use aggressive language toward children.
a. The tone of voice used by Staff #2 was described as " very aggressive, demanding, unkind, screaming and has scared children."
2. Staff #2 had a written coaching for her behavior on 01/13/2023 being in direct conflict with the center's "positive child guidance and respect for children policies."
3. Management confirmed Staff #2 had been verbally coached on her tone used with children on several occasions.

-Coach and retrain mandated reporting with all staff
-Management will hold staff meetings monthly
-Staff #2 was terminated based on licensing and internal investigation
Evidence:
1. On 01/13/20923 Staff #2 received a written coaching for pulling child #3 by the arm into the bathroom and screaming in his face for hitting her with a shoe.

Evidence:
Staff interviews confirmed staff #2 used remarks that were verbally demeaning to children in her care.
a. It was reported Staff #2 called a child weird.
b. It was reported Staff #2 called another child "retarded."
c. It was reported Staff #2 got in a child's face and screamed, "I am the boss."

Evidence:
The center had a menu posted in the front lobby that was dated for the following week. 05/13 - 05/17/2024. the current one-week period was not posted.
