Inspector Notes

An unannounced monitoring inspection was completed on 05/12/2025 as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 3:00pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. The inspector reviewed 5 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Prior to employment all staff are required to submit the results of fingerprinting. Staff B began work before receiving the results of a fingerprint.
Plan of Correction: This was an oversight. All staff are required to have fingerprints before starting to work.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. There cubby at the end of the changing table in the toddler room was peeling. There were remnants of stickers stuck to the floor in the infant room. An infant was observed sitting next to it and touching it.
Plan of Correction: The director will remove the stickers on the floor in the infant room and put in a work order to fix the cubby.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. The cabinet under the changing table was unlocked and had 3 bottles of disinfectant spray.
Plan of Correction: The staff locked the cubby with the chemicals during the inspection. The director will monitor for compliance.
Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied cots and rest mats.
There were several children during nap on cots that did not have 12 inches of space between them. Some cots were touching.
Plan of Correction: The director had recently spoken to staff about keeping cots spaced. she will follow-up again with all staff and will monitor for compliance that there is at least 12 inches of space between occupied cots.
Standard 8VAC20-780-450-A
Cots and rest mats are required to consist of a top and bottom cover. Three children in the Discovery Preschool room did not have a bottom cover on their cot during nap.
Plan of Correction: Extra sheets will be kept for children who forget both a top and bottom cover for nap.
