Inspector Notes

An unannounced monitoring inspection was completed on 09/18/2025 as a part of the licensure period. The on-site inspection began at 11:30am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements.There were 76 children present and 15 staff. The inspector reviewed 4 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and
prior to coming into contact with children. Staff #3 was hired in June of 2025 and did not have documentation of a tuberculosis screening until August 2025.
Plan of Correction: Due to certain circumstances involving the staff member's employment, the TB test was done after she started working. Staff are required to obtain a TB test prior to starting employment.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The center's last fire inspection was dated 01/29/2024.
Plan of Correction: The program director will contact the fire marshall to schedule a fire inspection. The follow-up call will be documented.
