This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review, and interviews.

A complaint inspection was initiated on December 12, 2021 and concluded on January 6, 2022. A complaint was received by the department regarding allegations in the areas of operational responsibilities. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.

The evidence gathered during the the investigation did not support the allegation of non-compliance with standards or law. No violations were issued.
