Inspector Notes

An unannounced complaint inspection was initiated on 09/07/2022 and completed on 09/08/2022 in response to a complaint received regarding care and protection of children and injury documentation. Based on the information gathered, which included a review of written records and staff interviews, the allegation regarding care and protection of children is found to be not valid. The allegation regarding injury documentation is found to be valid.

A violation was found in the area of special care provisions and emergencies and is listed on the violation notice issued to the facility.

Based on record review and interview, the center failed to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Evidence:
1. A review of written incident reports indicates that child 1 "kicked another student in the back two times with force" on 08/03/2022.
a. Staff 1 stated that she checked the back of the child and noted it was "a little red" and offered ice which the child declined.
b. No injury report was written for the child who was kicked in the back by child 1.
2. A review of written incident reports indicates that child 1 "punched another child in the chest" on 07/11/2022. A second incident report indicates that child 1"attacked a child physically" on 07/11/2022.
a. No injury report was written for the child who was punched or the child who was attacked physically.
3. Interviews with staff 1, staff 1, staff 2 and staff 3 confirmed that injury reports had not been written for the incidents occurring on 07/11/2022 and 08/03/2022.
a. Administrative staff confirmed that injury reports should have been written for these incidents.
Plan of Correction: Staff have been retrained on proper incident reporting procedures and requirements as of 9/9/22. Management will ensure that incident reports are completed for all children when incidents/accidents occur.
