Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 10, 2024 and completed on December 10. 2024. The on-site inspection began at 11:55am and ended at 2:17pm. The inspector reviewed compliance in the areas listed above. There were 55 children present and 9 staff. The inspector reviewed 6 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/23/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
A bag of cough drops in the pre- k room, a bottle of rubbing alcohol in the school age room and a bottle of toilet bowl cleaner in the early preschool restroom all had the warning labels, ?Keep out of reach of children? and were unlocked within reach of children.
Plan of Correction: All hazardous materials have been removed from classroom cabinets.
-The lock for the cabinet in the Pre-K classroom has been replaced with a functioning lock.
-The toilet bowl cleaner has been removed from the top of the bathroom cabinet and is now locked in the utility closet.
-The rubbing alcohol was removed from the cabinet and returned to the locked supply closet.

Documentation of at least two references to character, reputation, and competency are to be checked prior to employment.
Staff #1 who has been actively working at the center for the past two months had only one reference verification on file.
Plan of Correction: A second work reference was collected and verified for the employee. The director will ensure that all employees will have 2 verified professional references on file going forward.
