During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 8/1/25. The on-site inspection began at 11:20am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 82 children present and 17 staff. The inspector reviewed 9 children's records and 8 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-2
Centers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff 2 and staff 3, who have been employed for two months, did not have a completed national criminal background checks.
Plan of Correction: The center responded with the following: Staff 2 and staff 3 will schedule to have their fingerprints completed as soon as possible. They will not be permitted to work at the center until the search results have been received. Once obtained, the results will be forwarded to the licensing inspector.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a criminal history record check, a sex offender registry check or a search of the child abuse and neglect registry from one state for staff 2, who has been employed for two months.
Plan of Correction: The center responded with the following: The out-of-state background searches for staff 2 will be requested immediately. Once received, they will be placed in staff 2's record and forwarded to the licensing inspector.
Standard 8VAC20-780-70
Staff records shall include documentation that two or more references as to character and reputation as well as competency were checked before employment. The record for staff 1, who has been employed for over a month, contains only one of the two required reference checks.
