This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews , review of video footage and tour the building.

An unannounced complaint inspection was initiated on 06/20/2021 and concluded on 09/21/2021. The complainant reported concerns regarding the special care provision and emergency and administration, and staffing and supervision. An inspection was conducted at the facility, interviews were held with staff and documentation was obtained, and video footage was reviewed. The investigation was conducted jointly with a local agency. There is not a preponderance of evidence to support the allegation, therefore, this complaint has been determined to be Not Valid.
