This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a in person component.

A subsidy inspection was initiated on 11/19/2021 and concluded on 11/30/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Based on record review and staff interview, the licensee did not ensure the tuberculosis (TB) documentation was completed with in the last 30 calendar days of the date of employment.

Evidence:
Staff #1 hire date 8/12/2021 the tuberculosis (TB) screening was completed 11/2/2021.
Staff #3 hire date 7/19/2021 the tuberculosis (TB) screening was completed 04/28/2021.
Staff #4 hire date 9/27/2021 the tuberculosis (TB) screening was completed 7/15/2021.
