Inspector Notes

An unannounced monitoring inspection was conducted from 2:00pm-3:45pm. At the time of entrance, 34 children were in care with 7 staff members present. Additional staff were present. The sample size consisted of 4 children?s records and 3 staff records. Children were observed children waking up from nap, transitoning into snack and outside on the playground.

Information gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure the tuberculosis screening was completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: Staff #1 most recent TB test was 02/1/2023. Staff #1 employment date 03/20/2023. The TB test was not completed within the last 30days prior to employment.

Based on observation, the licensee did not ensure areas of equioment of the center, inside and outside be maintained in a clean , safe and operable condition.

1-On the preschool Playground, the playground composite there are multiple areas of peeling and flaking paint.

2- The gazebo ( shaded area ) the red poles multiple areas of peeling and flaking paint.

3- Inside the Pre-K room the window sill there is peeling paint .
