Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on June 29, 2023. The investigation was conducted jointly with a local agency. The inspection was initiated at the facility on July 11, 2023 and August 22, 2023 regarding allegations related to administration, staffing and supervison and programs. During the investigation,observation were completed at the center , staff were interviewed and documentation was reviewed.

There was not a preponderance of the evidence gathered during the investigation to support the allegation ; therefore, the allegation is determined to be not valid.
