Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on November 18, 2022 regarding allegations related to staffing and supervision, program and special care provision and emergencies. The inspection was initiated on November 30, 2022. During the investigation, video footage was reviewed, interviews with staff were completed and observations were made in the center, and documentation was reviewed.
There was a preponderance of the evidence gathered during the investigation support the allegation in area of programs; therefore, the program allegation is determined to be valid. In areas of special care provision and emergencies has been deemed not valid. Other violations were cited but were not reported in the initial complaint.

The head count sheets maintained by the center for the morning infant classroom revealed that the center did not meet the required staff-to-children ratio on the following date: January 19, 2023, from 7:56am to 8:30am; one staff with five children. Confirmed with Staff #3.

January 20, 2023, from 7:37am to 8:00 am; two staff with eighteen children; from 8:00 am
to 8:15am; three staff with nineteen children
January 24, 2023, from 8:10am to 8:35 am; three staff with 19 children
January 25, 2023, from 7:46am to 8:05am; three staff with 17 children; from 8:15am to
8:30am; three staff with seventeen children

Evidence:
Video footage from the two-year-old classroom on 11/29/2022, shows Child #1 put her hand in a toy bucket. Staff #1 abruptly pulls Child #1 by her bicep from the toy bucket. Child #1 walks
away and looks back to where Staff #1 is standing. Staff #1 walks over to Child #1 and abruptly pulls Child #1?s arm up by the bicep and puts Child #1 in the child size chair. Staff
#1 walks away. Video footage was reviewed with the Staff #5.
