Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint received 12/13/2022 regarding allegations forbidden actions and supervision of children. The inspection was completed 12/16/2022. The inspector was on-site from approximately 10:40 a.m. to 1:35 p.m. Staff interviews and review of documents were conducted for the complaint inspection.
The preponderance of the evidence gathered during the investigation supported the allegation. The complaint is determined to be valid.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

2Evidence:
According to interview, C2 age 2 was left alone on the playground on 08/08/2022 for approximately 22 minutes and the classroom teachers did not know that the child was left alone on the playground until the parent of the child reported seeing the child alone on the playground. The approximate temperature at 11:00 a.m. on 08/08/2022 was 85 degrees Fahrenheit. The incident report completed by the center documented that ?during the transition from the playground to the classroom C2 was separated from the group causing the child to remain on the playground. The report documented that actions that will be taken to prevent the incident in the future to include ?proper face to name procedures, checking playground?.
Evidence:
Interviews were conducted and supported S1 making verbal remarks which are demeaning to the child concerning toileting activities of C1. A review of documentation for S1 revealed S1 being ?re-trained on positive guidance and redirection.?
