Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on 04/04/2022 and concluded on 04/04/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 32 children present, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that staff member had a central registry finding within 30 days of employment.

Evidence:
1. Staff A date of hire 02/14/2022.
2. There was no documentation of a new central registry finding within 30 days of employment.
3. Administration confirmed the date of hire and there was not a new central registry finding on file.
Plan of Correction: I sent in the new central registry background check in for the staff member after our inspection.
Standard 8VAC20-780-160-A-2
Based on record review and interview, the center failed to ensure that staff member had documentation of a tuberculosis (TB) test/screening within the last 30 calendar days of the date of employment.

Evidence:
1. Staff A date of hire is 02/14/2022.
2. TB documentation on file dated 06/02/2021.
3. Administration confirmed the dates.
Plan of Correction: The staff member went on the date of inspection to complete a TB test/screening.
