Inspector Notes

A monitoring inspection was initiated on 10/18/2022 and concluded on 10/18/2022. There were 38 children present, ranging in ages from 2 years to 5 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records, 5 medication authorizations and 5 staff records were reviewed. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center did not ensure that staff had background checks completed every five years.

Evidence:
1. Staff # 5's Sworn Disclosure statement was originally signed on 09/22/2017.
2. There was not an updated Sworn Disclosure statement on file for Staff #5.
3. Administration confirmed that there was not a new Sworn Disclosure statement on file for Staff #5.
Plan of Correction: I am going to have the staff member complete a new Sworn Disclosure statement today before she leaves work. I will go through the files to make sure that no one else has any background checks getting ready to expire.
