Inspector Notes

An unannounced inspection was conducted on February 29, 2024 from 1:35 p.m. to 2:35 p.m. in response to a self-report regarding biting. Classroom observation was conducted. Staff interviews were conducted. Documentation was reviewed including incident/injury reports and one child's record.

Additional documentation was reviewed off site.

An exit interview was conducted with the director by phone on April 17, 2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it within five (5) calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s)

Based on staff interviews and review of documentation, it was determined that while supervising children, staff failed to ensure their care, protection and guidance.
Evidence: From September of 2023 to February of 2024, Child A, currently two years old, bit other children at least 42 separate times. Some of the bites broke skin and caused bleeding. Some bites required first aid intervention such as ice.
