Inspector Notes
An unannounced inspection was initiated on 02/18/2026 and completed on 03/04/2026 in response to a complaint received by the licensing office on 02/05/2026 relating to allegations of staffing and supervision and physical plant. This inspection included: tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
