Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 2:40pm-3:35pm in response to self-reported incident. Observations were made, documents were reviewed, and staff were interviewed. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations
4Evidence:
1) Staff A fed Child A food that listed on the allergy plan of Child A.
2) After a conversation with the parents, Staff B stated that Child A vomited twice once they were home.
Evidence:
Staff B stated that the infant room does not have a lead teacher regularly present.
Evidence:
The record of Child A stated that Child A was allergic to rice, oats and bananas. These items were missing from the most recent allergy list.
-Will set calendar reminder to update lis monthly, as well as with each time a new allergy is noted with a child.
Evidence:
1) The record of Child A states Child A is allergic to oats and bananas.
2) Staff A stated that she served Child A three tablespoons full of oatmeal and three bites of banana.
