Inspector Notes
An unannounced renewal inspection was conducted today from approximately 11:50am-1:40pm. There were 77 children in the direct care and supervision of 16 staff. A sample of 5 children?s records, and 5 staff records were reviewed. The children were observed napping, singing, eating and during bathroom procedures. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations
4Evidence:
1) Staff B was missing a complete out of state central registry search, and a complete criminal history search.
Evidence:
1) Staff A completed 14.5 of the 16 required annual training hours.
Evidence:
Of 9 minor injury reports reviewed the following were missing:
1) 5 were missing time the parent was notified of the injury.
2) 5 were missing the method in which the parent was notified of the injury.
3) 3 were missing the future action to prevent the injury.
Evidence:
3 of 5 children's records reviewed were incomplete.
1) The record of Child A was missing their father's employer and employer phone number.
2) The record of Child B was missing the address of one emergency contact.
3) The record of Child C was missing the mother's employer phone number and the father's employer and employer phone number.
