Inspector Notes

An unannounced investigation was conducted 03/28/2023 in response to a complaint regarding supervision and care of children. During the investigation staff were interviewed, the files of children and staff were reviewed, and the "Toddler's" classroom were observed. The preponderance of evidence supports the allegations. This complaint has been determined to be valid. The exit interview was conducted with the director. Standard 8VAC20-780-340-A
Based on interview and photos reviewed, When staff are supervising children, they did not always ensure their care, protection, and guidance.

Evidence:
1) On February 13th 2023, Child A (2 years old) fell off of the climbing equipment in the ?Toddler?s? classroom and their lip began to bleed. Neither Staff administered First Aid.
2) Staff B confirmed that Child A began to cry and their lip was bleeding.
3) Staff B stated Staff A told her not to tend to Child A.

Standard 8VAC20-780-410-1
Based on interview, staff have roughly handled a child.

Evidence:
1) On March 1st 2023, Staff A pushed Child A onto a chair to prevent Child A (2 years old) from going to the door. Child A began to cry.
2) Staff B stated that they witnessed Staff A push Child A onto the chair located near the door of the ?Toddler?s? classroom.
