Inspector Notes
An unannounced investigation was conducted in response to a complaint regarding care of children. During the investigation staff were interviewed, the files of children and staff were reviewed, and classrooms were observed. The preponderance of evidence does support the allegations. This complaint has been determined to be valid. The exit interview was conducted with the director. Violations
4Evidence:
1. The tuberculosis exam of Staff A expired 07/2023 and was not repeated.
2. The tuberculosis exam of Staff B expired 08/2023 and was not repeated.
Based on interview, when a child's clothing or diaper became wet or soiled, the child was not cleaned and changed immediately.
Evidence:
1. On August 2nd Child C was dropped off at approximately 9:30am. The Child was picked up approximately 7 hours later and was found in soiled diaper and clothes.
2. A staff confirmed that the teachers in the classroom did not change the diaper of Child C all day.
Evidence:
Emergency numbers were not posted near the telephone located in the office.
Evidence:
1. The record of Child A and B were missing the father's employer.
