Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 07/30/2024, as a part of the licensure period. In addition a self-report was received on 7/22/2024 involving infant feeding produres. The on-site inspection began at 9:50 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 110 children present and 20 staff. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review (i.e. policies and procedures, emergency drill logs, authorization forms, child files and staff files), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/5/2024. A POC submitted after this date will not appear on the public website.
Violations
4Evidence:
1. In the Waddlers classroom, there was an end cap on the crash bar of the playground door that was falling off, leaving a gap that posed a hazard for pinched fingers.
2. On the preschool playground, an exterior screen was ripped.
Evidence:
1. Cleaning materials were on a lower shelf in the changing table cabinet in infants, within reach of a mobile infant.
2. A changing table cabinet in the toddler classroom containing cleaning materials was left unlocked.
Evidence:
On the date of the misfeed incident (07/22/2024) the center's bottle-feeding policy includes a two-step verification process that includes one staff member to verify the name on the bottle and a comparison to the child being fed, as well as a second staff member verifying that the correct bottle is being fed to the correct child. Staff #6 did not perform this step, or ask staff #4 to verify the correct bottle was being given.
Evidence:
In the Discovery Preschool classroom a lunch box with food inside was not labeled with the child's name or today's date.
