During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 03/10/2026 in response to a self-report received by the licensing office on 03/09/2026 relating to supervision and behavior guidance. The inspector reviewed 1 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/18/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 03/09/2026, Child #1 was left on the playground unattended, therefore Staff #1, #2, and #3 were not available to ensure proper care, protection and guidance.
Plan of Correction: Staff retrained on playground safety plan ensuring all children are in sight and sound.
Standard 8VAC20-780-340-F
Children under 10 years of age shall be within actual sight and sound supervision of staff.

Child #1 (3 years of age) was left unattended on the playground, outside of both sight and sound of assigned Staff members #1, #2, and #3.
Plan of Correction: Reviewed VAC20-780-340 Supervision of children with all staff.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior.

On 03/09/2026, after Child #1 was left unattended on the playground for approximately 8 minutes, Staff #1 proceeded to scold Child #1, shouting "I'm mad at you, why did you do this?"
Plan of Correction: Teachers retrained on Positive Guidance policy. All staff retrained on Child supervision record and name to face transitions.
Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: roughly handling a child.

On 03/09/2026, Staff #1 roughly handled Child #1 by holding and squeezing their face while after the child had been left unattended on the playground.
Plan of Correction: Staff 1 violated the center's positive guidance policy and was immediately removed of caring for children in the classroom. Staff 1 has since been terminated.
