Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 3:15pm and 4:25pm. There were 6 classrooms observed with a total of 66 children with 13 staff within the supervision guidelines. The children were observed eating snack (crackers, applesauce, and water), having circle time and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard 8VAC20-780-160-A
Based on a review of staff records, 3 out of 4 staff did not submit documentation of a negative tuberculosis screening that was completed within the last 30 calendar days of the date of employment.

Evidence:
1. Staff D (doh: 8/16/2022) had a tuberculosis screening on file dated 4/9/2022.
Plan of Correction: We will make sure staff submit a TB screening in the allotted timeframe.
Standard 8VAC20-780-240-A
Based on a review of staff records, 1 out of 4 staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff D (doh: 8/16/2022) did not have documentation of completing the Virginia
Department of Education-sponsored orientation course on file at the time of the
inspection.
Plan of Correction: The staff member will complete the required 10 hour training by this Friday.
Standard 8VAC20-780-240-E
Based on a review of staff records, 1 out of 4 staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, within 30 days of the first day of employment.

Evidence:
1. Staff D (doh: 8/16/2022) did not have documentation of training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, on file during the time of the inspection.
Plan of Correction: We will get the staff member the required first aid/cpr training as soon as possible.
