Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on May 20, 2021 and concluded on May 20, 2021. The Provider or was contacted by telephone to initiate the inspection. There were 10 children present and 3 staff for a total of 25 points. The Inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility or the program. Please call with questions 703-479-4679

Based on review of the staff files, it was determined two references were not contained in Caregiver #2's record.
Plan of Correction: I will place them into the file.
Standard 22VAC40-111-180-A
Based on interview and record review, it was determined the Provider was unable to provider documentation of Caregiver #3's updated TB Screening.
Evidence:
1. Provider stated Caregiver 2 submitted her updated TB Screening to her but she is unable to locate and show the Inspector.
Plan of Correction: I will locate the documentation and place it in the file.
