Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documents review, interview, and a virtual component tour of the program.

A monitoring inspection was initiated on December 1, 2021 and concluded on December 3, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 11 children present, ranging in ages from infant to 3 years, with 2 staff supervising for a total of 30 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The Provider sent documentation requested for child records and staff records. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interview and record review, it was determined the Provider did not maintain in caregiver's 1 record documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
1. Provider stated she had a new assistant that started in early November. The Provider stated she didn't have documentation of 2 references.
Plan of Correction: I will have her provide me with two written references in the next week.
