Inspector Notes

An unannounced monitoring inspection was conducted on 12?18/2023 from 11:15 to 12:05.
The provider and an assistant were present with 6 children totaling 16 points. The children were observed playing , eating lunch and watching a show on tv before they went down for nap. Lots of interaction between the caregivers and children. Todays lunch was peanut butter and jelly sandwiches and bologna sandwiches. milk and bananas. The provider has lots of toys for the children to play with. A violation notice was given to the provider along with this inspection summary. Standard 8VAC20-800-180-A
Based on the review of the caregiver records, the provider failed to get a subsequent TB screen for the assistant.
Evidence: The assistant has a TB test that expired on 10/5/23.
Plan of Correction: We were confused because she had to take antibiotics before and have had a vaccine for TB, we understand now she can get a screening and will send her to get that done.

Based on the review of children's records, the provider failed to get some of the information needed to make the records complete.
Evidence: 2 out of 4 children's records reviewed are missing the second emergency contact.

Plan of Correction: I am the emergency contact for one of the children and I will ask the other parents again for one.
