Inspector Notes

An unannounced monitoring inspection was initiated on 7/31/2025 and completed on 8/21/2025 with a local agency, in response to a self-report received by the licensing office on 7/28/2025 relating to forbidden actions. The inspector reviewed 1 child?s records and 1 staff record on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to injury reporting and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/27/2025. A POC submitted after this date will not appear on the public website.

The center shall maintain a written record of children's serious and minor injuries
in which entries are made the day of occurrence.

On 7/14/2025, Child #1 was given a bandage for an injury to their middle finger by Staff A. Staff B stated that when they asked Child #1 what happened, Child #1 stated that it hurt. Staff B reported that they observed a little blood on the bandage and assumed it was a hangnail. Staff C reported that they observed Child #1?s finger and described it as a scratch. An injury report was not completed for Child #1?s injury on the day of occurrence.
Plan of Correction: 1. Director spoke with Staff A & Staff C (Staff B is no longer employed) regarding the importance of documentation.

2. Director sent all staff an email stating, " All serious and minor injuries that occur to a child in our care on site must be documented using an accident form on the day of the incident.
